901 KAR 5:060. Birth certificate reissued; circumstances

Section 1. Issuance of New Birth Certificates Under Certain Circumstances. The Director of Vital Statistics may issue a new birth certificate where the original certificate is mutilated, defaced or contains information of a personal sensitive nature which might embarrass the applicant and which is not pertinent to the vital information required in the certificate. NECESSITY, FUNCTION, AND CONFORMITY: KRS Chapter 213 relating to Vital Statistics authorizes the Cabinet for Human Resources to regulate the registration of births in Kentucky. The purpose of this administrative regulation is to provide a uniform procedure for issuing a new birth certificate where the old certificate is mutilated, defaced or contains information of a personal sensitive nature which might embarrass the applicant and which is not pertinent to the vital information required in the certificate.
